Lizanne has worked as a Case Manager since 2014, specialising in amputees, complex trauma, spinal injury and brain injury. Lizanne has successfully supported and managed complex clients and their families through their rehabilitation to ensure they receive the services that they are entitled to and achieve their maximum level of independence and function.

Lizanne is an excellent communicator and very person-centred. She is able to build therapeutic relationships with her clients, their families, carers and other professionals to ensure the best possible outcome. Lizanne is sensitive to the needs of her clients and their families as they adjust to their potentially life-changing injuries.

Lizanne has organised, co-ordinated and managed complex packages of care within the NHS and private sector and has the ability to be flexible and adaptable to meet the individual needs of the client to ensure that they receive the right level of support, provided in an environment that is able to meet their needs.

Lizanne qualified as an Occupational Therapist in 1994 and has expertise in complex trauma, brain injury and neurological conditions. Working for the NHS and local authority in an in-patient and community setting.

Having worked in the NHS for over twenty years, Lizanne also has expertise in supporting complex clients with their discharge from hospital; ensuring that the patient is at the centre of the decision-making process and the discharge is as seamless as possible.

Lizanne carries out detailed holistic assessments with all of her clients. Working with the clients, their families and carers to produce realistic client-centred rehabilitation goals. Lizanne also compliments her case management role by working as a Care Expert.
